The
Neumann Scholarship Foundation is a program derived from the state Educational
Improvement Tax Credit plan, which allows companies to take a credit against
certain state taxes in return for their donation to qualified scholarship
organizations. Companies may make an
unrestricted donation or designate their donation for a particular school. For
the 2008-2009 school year, $135,000 has
been earmarked for tuition assistance for students attending Our Lady of the
The Diocese works with
PSAS, the company that has been selected to manage the program.
Please
complete the attached worksheet to determine your eligibility. These worksheets
are for your personal use and are not returned
to the office. Applications for the
scholarship will be available upon
request at the school office. Completed applications are sent to PSAS,
who will verify the information, process and retain records, as required by
state law. PSAS will notify families of
any scholarship award sometime in June.
To assist you in this
application process:

Complete
the worksheet attached to this flyer to see if your family will meet the state
guidelines. This is only a worksheet; the
application must be requested from the office.
¨
If you have
determined your family may qualify, please request an application from the
school office. Complete application
including the Educational Improvement Tax Credit Scholarship
Guidelines/Instructions page. This must accompany
the completed scholarship application form.
¨
One
application is used per family, even if students attend different Catholic
Schools in the Diocese.
¨
Use the
correct SCHOOL CODE: 10307
¨
The fee, all
tax forms and information must be provided.
Incomplete applications may not be able to be processed. Both Federal and State Tax forms are
required. If you do not file the PA tax
form, a copy of a utility bill may be submitted as proof of residency.

Fee must be
included with the application.
¨
Completed
forms are mailed in the envelope provided to PSAS.
¨
Families will
be notified by PSAS of scholarship awards sometime in June.
¨ The deadline for applications is April 1st.
¨ No late forms will be accepted.
